100% accuracy moves missing in generation 2

In the first generation, when the game determines whether a move hits or misses, it calculates the number X. Then, it generates a number Y. The move is supposed to hit, when Y is lower than or equal to X. But it only hits if Y is lower than X. But as far as I know, that glitch was fixed in generation 2, but for some reason, 100% accuracy moves still miss sometimes. Does anybody have an explanation why this happens? Thanks.

Are you sure the target wasn't holding Brightpowder?

I've seen this happen too in-game, at least for non-damaging moves like Sand-Attack. It seems to happen more than .4% of the time, too. And since I've seen it numerous times in-game, and wild Pokémon/trained Pokémon in-game don't often hold items (much less rare items), I doubt it's BrightPowder at work…
