You eat the barf, giving you a power up. As you do, the dead fish appears on the door handle, and turns into a 1- up mushroom.

You are now in a ROOM with a DOOR, a CUSTOM NUKE, and a 1-UP MUSHROOM

You get the KNIFE, and suddenly the GLITCH in the corner goes berserk and breaks out the window, running for the hills! The PROFESSOR blames you for scaring it with that KNIFE. Looks like you'll have to go catch it and bring it back…

You are in a LAB with PROFESSOR OKK (who is currently mad at you), some very nice BUNSEN BURNERS, and a broken window. Obvious exits are SOUTH and WINDOW.
